What the issuer is asking

The code alone does not tell you what to prove. The notice carries a sub-condition, and that is the line that matters.

Find it before you gather anything. Goods not provided needs delivery; not as described needs the listing; cancelled recurring needs the cancellation record.

Mastercard also expects the cardholder to have contacted you first. If they did not, that is part of your answer.
